“BATWatch™” refers to BATWatch Medical Group, LLC, its affiliated series entities, and any state-licensed professional corporations or entities operating under the BATWatch™ system (collectively referred to as “BATWatch™”).
You authorize BATWatch™ and its representatives to contact Your primary care provider and/or any other relevant healthcare professionals handling Your care now, or in the future, to share educational information and testing data related to Your BAT Levels and brain health support plan.
You authorize BATWatch™ to share relevant lab reports, summaries, and protocol information with Your primary care provider and/or any other relevant healthcare professionals handling Your care now, or in the future, to support coordination of care.
You understand that BATWatch™ may or may not currently be a treating provider in Your state. When not licensed, all communication is provided for educational and coordination purposes only. If and when BATWatch™ is licensed in Your state, You understand that direct services such as testing, treatment guidance, and follow-up may be offered under applicable regulations.
Scope of Support & Disclosures
You understand that any guidance or recommendations provided by BATWatch™ in non-licensed states are for informational and educational purposes only, and that all medical decisions will be made by Your licensed primary care provider and/or any other relevant healthcare professionals handling Your care now, or in the future.
You understand that BATWatch™ does not replace Your current care team but serves as a centralized brain health tracking system and support partner.
HIPAA Authorization & Confidentiality
You authorize BATWatch™ to send and receive protected health information (PHI) as needed to support coordination of care, including lab results, reports, and relevant clinical information.
You understand that all information shared will be handled in accordance with HIPAA and relevant data privacy laws.
Instruction to Providers for Full Data Sharing
You instruct Your primary care provider and/or any other relevant healthcare professionals handling Your care now, or in the future, to fully cooperate with BATWatch™ and to release all relevant testing, imaging, and clinical records related to Your brain health and biological testing. This includes sharing results, reports, and any related communications necessary to support longitudinal tracking, care coordination, and inclusion in the BATWatch™ Registry.
Your understand that this data will be used solely to improve patient outcomes, identify patterns, and support longitudinal cognitive care through the BATWatch™ system.
Anonymized Registry & Research Data
You understand this authorization remains valid for 12 months unless You revoke it in writing. You may revoke this consent at any time by notifying BATWatch™ in writing at careteam [at] batlevels.com. Revocation will not apply to any information already shared prior to receipt of the revocation notice. However, de-identified health data may be retained indefinitely under the separate research authorization below.
You understand BATWatch™ maintains a secure registry of anonymized testing & treatment data. This registry helps improve care protocols, identify trends, and support future research in the field of brain health. No personally identifiable information will ever be used or shared for research purposes. Data will be de-identified in accordance with HIPAA and stored securely and used solely for ethical research and protocol development within the BATWatch™ system.
You authorize BATWatch™ to retain and use Your anonymized health data, including biomarker trends, treatment response, and contributing factors, for research, clinical quality improvement, and the advancement of brain health science.
Acknowledgment
By requesting to participate in the BATWatch™ program, You confirm that You have read, understand, and voluntarily authorize BATWatch™ to coordinate with Your primary care provider and/or any other relevant healthcare professionals involved in Your care now or in the future, whether listed here or not, and to include Your relevant clinical data in the BATWatch™ Registry, as described above.
